We will be taking applications through November 24, 2021

Gift for Children: Birth – 12 years

 

PLEASE NOTE YOU WILL NEED TO BRING: 

Picture ID for Adults in the home

Birth Certificate or Medical Cards with DOB for all children

Proof of Residence

Proof of Income
